Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.

People were supported to have equity in experiences and outcomes. The nominated individual told us that people were supported to access community based facilities. These included health services, places of worship and leisure activities. We saw that care plans covered people’s needs in these areas. Care plans also covered needs related to equality and diversity and there was a policy on this to help guide staff in their practice.
