Assessing needs
Feedback from people was mostly positive. People felt involved in their assessments and felt confident staff understood their individual and cultural needs. Patients we spoke with told us they wanted open access back. For patients this meant they would wait for an appointment with a doctor on the day. Patients told us this gave them the opportunity to chat with each other.
Staff told us they wanted open access back where patients would come and wait for a doctor’s appointment. Staff told us this happened before the pandemic and the community spirit they felt had been lost. Staff told us they had a community vegetable garden led by one of the doctors. The vegetables and fruit were available for patients to take as they needed. Leaders told us they would review open access as part of their triage and appointment system review. Leaders acknowledged there had been changes since the pandemic and staff had faced challenges working differently. Leaders continued to work with community groups and were supported by the patient participation group.
The provider had systems and processes in place to keep clinicians up to date with current evidence-based practice. People’s immediate needs were assessed. This included their clinical, mental and physical wellbeing. Staff added digital flags within the care record system to highlight any specific individual needs, such as the need for a translator or longer appointment time.
Delivering evidence-based care and treatment
87.5% of patients from the NHS England GP Patient Survey results stated that during their last appointment, the healthcare professional was very good or fairly good at treating them with care and concern. 95.2% of patients from the NHS England GP Patient Survey results stated that during their last appointment they had confidence and trust in the healthcare professional they saw or spoke to.
Staff and leaders told us they took a holistic approach to delivering evidence-based care and treatment. Patients with complex diabetes were referred for lifestyle and dietary support following no improvement from taking prescribed medication. The blood results were dramatically improved following a 12-week intensive lifestyle programme.
There were processes and procedures for escalating concerns and to ensure patients needs were met. The service worked with the Primary Care Network to bench mark performance. We found the service followed national guidelines and best practice, but this was not audited routinely.
