Safe
Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question requires improvement and identified a regulatory breach in relation to the governance of medicines management systems. At this assessment improvements had been made and the rating has changed to good. This meant people were safe and protected from avoidable harm.

Learning culture
The regional manager and registered manager had worked to create a proactive and positive culture of safety, based on openness and honesty. Concerns about safety were listened to, reported and investigated. Lessons were learnt following audits, incidents, and assessments of sister homes to continually identify and embed good practice.

Involving people to manage risks
The registered manager worked with people and staff to understand and manage risks by thinking holistically. Risks were identified and assessments completed to ensure staff provided safe care in relation to areas such as falls management, wound care, nutrition and continence care. There was a team approach to managing risks, which included discussions at handovers and, regular monitoring and reviews. People, or their relatives, were appropriate, were involved in discussions around their care and support.

Safe and effective staffing
There were enough staff, who were appropriately deployed to make sure people’s needs were met. Safe recruitment practices were in place. However, we identified some shortfalls in the recording of Disclosure and Barring Service (DBS) checks. The management team accepted our feedback and made immediate improvements. The registered manager made sure staff received support, supervision and development opportunities. Staff worked well together to provide safe care which met people’s individual needs.
Infection prevention and control

Medicines optimisation
Medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. Medicines were stored securely; the treatment room was clean and tidy and temperature monitoring was in place. Staff knew people well and could describe how they supported them with their medicines. People received their medicines as prescribed, including time critical medicines. However, further work was needed to ensure records for medicines administered by a patch, enabled staff to rotate applications in line with manufacturers recommendations. Protocols and care plans were in place to support staff in the safe administration of medicines. Where multiple medicines of the same type were prescribed more detail was required so staff knew which to use first. The oversight of medicines was improving. Alongside internal audits, regional and independent audits had taken place. Where points had been identified for improvement, these were being worked on. Where errors had occurred, a review was completed so lessons could be learnt.
