Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. Staff knew how to report accidents or incidents. These were reviewed by the manager and actions taken to reduce the risk of a reoccurrence. Incidents were discussed as a team and with people’s families, if appropriate, to identify any new triggers or strategies to support people in future. All incidents, including lower-level ones, were logged so patterns or changes in behaviours could be identified.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. The service worked with education services, healthcare professionals, social services and commissioners in a transparent and appropriate way, that supported peoples individual care needs. Issues between learning disability and mental health services as to which service was responsible for taking the lead in 1 person’s support were resolved by the service.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. People and their representatives were involved in all aspects of their care. The staff and leaders had taken active steps to ensure there was an open culture. Staff said they were able to report any concerns, and these would be acted on. People’s sexual safety was considered, and their sexual health and needs were considered and supported.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. People were supported and empowered to take risks in areas they wanted to and to enhance their lives. Potential risks were identified, and guidance written for staff to minimise these risks. These were regularly reviewed and updated when people’s needs changed.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. Regular health and safety checks were completed, and all equipment was serviced in line with legal guidelines. Any maintenance issues were completed in a timely way. The environment met the needs of the people living at the service.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. People were supported by enough staff, who had undertaken training in how to meet their specific needs. People’s individual needs and care plans were understood by staff. Staff competencies in a range of areas were assessed. Staff had been safely recruited and said they were supported in their role through regular supervision and team meetings.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. The home was visibly clean throughout. Cleaning schedules were used to ensure all areas were regularly cleaned.

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. People received their medicines as prescribed. People’s support needs for their medicines were recorded in their care plans. Clear procedures were in place to ensure all medicines were provided, and recorded, when people visited their families. Daily and weekly medicines checks were completed.
